CCDC Registration Form
Registration forms should be completed and brought to the center along with a $25 non-refundable deposit in order for the registration form to be processed.

Written Medical Consent Form
We encourage parents to issue medications at home whenever possible. If medications need to be administered at the center, please complete the written medication consent form and bring it with the medication to the center.
ALL MEDICATION MUST BE IN ITS ORIGINAL CONTAINER.
